Providence Health & Services Physical Therapist in Centralia, Washington
Providence is calling a Physical Therapist (0.8 FTE, Days) to Providence Centralia Hospital in Centralia, Washington.
We are seeking a licensed PT to join our team of caregivers who provide therapy to patients to restore function, relieve pain, and prevent disability.
In this position you will:
Consult with the physician and an interdisciplinary team to perform PT evaluations, establish treatment plans with goals, provide treatment, delegate specific treatments and perform discharge activities. You may also be involved with in and out-patient would care along with neuro out-patients.
Provide education to patients and their families.
Utilize software systems for the maintenance of patient records and communication with team members.
Required qualifications for this position include:
Bachelor’s Degree and a Graduate of an accredited school of Physical Therapy
Current Physical Therapy (PT) licensure in Washington State
Current BLS certification
Preferred qualifications for this position include:
- Post graduate experience.

This rehab department is a multi-disciplinary team of PT/OT/SLP and front office staff. Our staff range in experience from one year to 30 . Many of the therapists have been with Providence Centralia for over 20 years therefore bringing many years of experience and expertise in areas of general acute care, wounds, neuro, hands and pediatrics.
